Background images are only visible in the Left/Right/Top/Bottom/Front/Back and Camera views, it doesn't work in Perspective view.

What do you mean by that? "Load Factory Settings" just resets the interface, you can always open your .blend file after that...

Why there isn't a "reset all menus to default" button is beyond me. File -> Load Factory Settings If nothing is drawn on the menu, that usually indicates a Python problem (can't load the layout). Check the console for python errors. Also, until you fix your problem, you can call the "Redo Last" fun...

That's done with the 3D Cursor - you have to move where you want, and then snap the origin to the 3D Cursor. You can also use the 3D Cursor as a rotation pivot:

In the Textures tab, under Image Sampling, disable MipMap and Interpolation. You can also set the Filter Size to the lowest possible (0.10) for a sharper result.

1. Objects in blender aren't parametric like they are in 3dsmax. The polycount can be adjusted only once, right after you create the object. Every function's parameters can be altered once after you execute it, and the "add object" function is no exception. After adding an object, look at the bottom...

It depends on which version you're using, since the UI has been reorganized in 2.5x+ to make more sense. You have to assign the texture you painted onto the shoe's material if you want it to appear when rendering.
