Posting a .blend might have been easier, as you never know what might get lost in translation between formats...
After importing, I saw a few verts in the arm and the head that are not weighted properly. When I move the bones, the model moves correctly, but those verts remain frozen in space.
I also performed a 'remove doubles' on the mesh and found 73 duplicated verts. It is possible that you selected a duplicate vert during weight painting, and the other remained unweighted and frozen.

Thrasha, thanks for pointing out that there were double vertices, I didn't notice. I removed doubles and re-painted with meticulous attention to detail. The distortion isn't as bad, but is still a very large problem. This is what I am currently dealing with: http://imgur.com/zDugX
I put an IK constraint on the bone with a chain length of two. My main concern is that the mesh does not follow the bones. Even with the IK constraint, the mesh moves around and away from the bone.
